چکیده

Introduction Thrombotic microangiopathy is a term for a morphologic lesion characterized by platelet and fibrin thrombi involving the microvasculature. The morphologic lesions thus overlap between hemolytic syndrome (HUS), thrombotic thrombocytopenic purpura (TTP), and the lesions seen in scleroderma and malignant hypertension. HUS/TTP typically involves glomeruli and smaller vessels, whereas scleroderma and malignant hypertension involve interlobular arteries and arterioles. Hemolytic uremic syndrome (HUS) and thrombotic thrombocytopenic purpura (TTP) share the morphologic lesion of thrombotic microangiopathy, characterized by platelet thrombi occluding the microvasculature. The HUS and TTP syndromes overlap clinically. Recent evidence indicates differing pathogenesis (see below).

Thrombotic Microangiopathies

Thrombotic microangiopathy results from thrombotic occlusion of the microvasculature leading to fragmentation of red blood cells, profound thrombocytopenia, and a microangiopathic hemolytic anemia with elevation of lactate dehydrogenase and negative direct Coomb's test. This constellation of clinical and laboratory findings is not due to one disease entity; rather, it represents a variety of un...

Successful antemortem diagnosis and treatment of pulmonary tumor thrombotic microangiopathy.

Pulmonary tumor thrombotic microangiopathy (PTTM) is a fatal cancer-related complication with rapid progression. Although the underlying molecular mechanisms of PTTM remain unclear, platelet-derived growth factor (PDGF) and vascular endothelial growth factor (VEGF) may play important roles in the pathogenesis of PTTM. We herein report a case of PTTM that was diagnosed antemortem by pathology us...

Pathogenesis of thrombotic microangiopathy: insights from animal models.

Animal models are important experimental tools for investigating the molecular mechanisms, environmental and genetic susceptibilities underlying the development of thrombotic microangiopathies. Large mammal, small animal models, knockout, transgenic and conditional knockout mouse models are available to investigate haemolytic uraemic syndrome, thrombotic thrombocytopenic purpura and vascular en...

THROMBOTIC MICROANGIOPATHY ASSOCIATED WITH OPANA ER INTRAVENOUS ABUSE A Case Report.

Thrombotic microangiopathy is characterized by endothelial changes and microvascular stenosis. Several entities such as pregnancy, infection, connective tissue diseases, and drugs are associated with secondary thrombotic microangiopathy. Recently, new reformulation of Opana ER had been associated with thrombotic microangiopathy when injected intravenously. Here, we report the case of a 37-year-...

Update on the role of the complement system in the pathogenesis of thrombotic microangiopathies.

In thrombotic microangiopathies (TMA) pathological changes of the small vessels are present, which lead to ischaemia of the affected tissues, low platelet-count and intravascular haemolytic anaemia with fragmentocytes. Two main clinical syndromes belong to the group of TMAs: the haemolytic uraemic syndrome (HUS) with kidney failure, mainly affecting children, and the thrombotic thrombocytopenic...
